Fatigue on Mounjaro and Ozempic: Why You Are So Tired, and What Helps

Last reviewed August 2026 · by the Sated team · not medical advice

The short answer: fatigue on a GLP-1 is common and it is usually the sum of ordinary things rather than one mysterious side effect. You are eating far less than you used to, which means fewer calories, less protein, less iron and less fluid. On top of that, side effects often cluster in the 48 hours after a dose, when the medication is at its peak. Most people can move the needle by fixing intake and hydration first, and the rest is worth raising at your next review.

The most likely reason is the simplest one

These medications work by making you much less hungry. That is the point. But appetite is also the mechanism that used to make you eat enough, and when it goes quiet, intake can fall a long way below what your body actually needs without you noticing. People routinely report eating a third of what they used to and feeling fine about it, right up until the afternoon they cannot keep their eyes open.

Very low calorie intake produces fatigue on its own. It is not a sign that something has gone wrong with you, and it is not a reason to push through. It is a sign that the amount you are eating and the amount you need have come apart.

Protein is the piece people miss

Protein matters here for two reasons. The obvious one is muscle: when you lose weight quickly, some of what you lose is lean tissue, and protein is the main thing that limits how much. The less obvious one is that losing muscle lowers the energy your body burns at rest, which makes everything feel harder over time.

Most guidance for people losing weight lands somewhere around 1.2 to 1.6 grams of protein per kilogram of body weight per day, against the 0.8 figure often quoted for the general population. You can get your own number from our GLP-1 protein calculator, and there is a fuller explanation in how much protein you need on a GLP-1.

The practical problem is that protein is filling, and you are already full. That usually means front-loading it: getting a meaningful amount early in the day, while your appetite is at its best, rather than hoping to catch up at dinner.

Dehydration is the quiet one

When you eat less, you drink less without meaning to, because a surprising share of daily fluid arrives inside food. Add any nausea, vomiting or loose stools and the gap widens quickly.

Mild dehydration produces exactly the symptoms people attribute to the medication: tiredness, headache, poor concentration, feeling faint on standing. It is worth ruling out first precisely because it is the easiest thing on this list to fix. There is more in our guide to hydration on a GLP-1.

The 48 hours after your dose

Plenty of people find their worst days are clustered right after an injection, when the drug level in the body peaks, and that things ease off later in the week. Others find the opposite, feeling flattest in the days just before the next dose is due.

Neither pattern is a problem in itself, but knowing which one you have is genuinely useful, because it lets you plan around it rather than being surprised by it every week. This is the sort of thing that is almost impossible to hold in your head and obvious the moment it is written down: log how you feel for a few weeks and the shape usually appears on its own.

Things worth ruling out with a clinician

Some causes of fatigue are not about the medication at all, and a few are worth checking properly rather than guessing at:

If tiredness is severe, getting worse, or comes with dizziness, a racing heart or breathlessness, that is a reason to speak to someone promptly rather than wait for a scheduled review.

What to try first

In rough order of how often it works:

One thing not to do is change your dose to see if it helps. That decision belongs with the person who prescribed it, and there are usually several cheaper things to try first.

Fatigue often tracks the dose cycle. The dose schedule calculator shows where the standard increases fall, and the injection site guide covers the practical side of dose day.

Common questions

Is fatigue a normal side effect of Mounjaro?

Tiredness is commonly reported by people taking tirzepatide and semaglutide. It is usually explained by sharply reduced food and fluid intake rather than by the medication acting directly, which is why fixing intake often helps.

How long does tiredness last on a GLP-1?

For many people it is worst in the first few weeks and after each dose increase, then settles as the body adjusts. If it persists or worsens, that is worth investigating rather than waiting out.

Can low protein make me tired on Mounjaro?

It contributes. Protein protects muscle during weight loss, and losing muscle lowers the energy you burn at rest, which makes daily activity feel harder over time. Guidance for people losing weight commonly sits around 1.2 to 1.6 g per kg of body weight per day.

Should I stop taking it because I am so tired?

Never change or stop a prescribed medication on your own. Tiredness is usually manageable and has several common causes worth addressing first. Speak to your prescriber, and go sooner if it is severe or comes with dizziness, breathlessness or a racing heart.

Could it be iron deficiency rather than the medication?

It could, and it is worth asking about. Eating substantially less, particularly less meat, can reduce iron intake, and low iron causes this kind of fatigue. It needs a blood test to identify, so it is a question for your GP rather than something to guess at.
